To schedule an appointment, call our office at 304.344.3551. We ask that you arrive fifteen minutes prior to your appointment time in order to be seen promptly. Unfortunately, emergencies can arise that may cause us to fall behind schedule. We appreciate your patience.

To expedite the registration process, please use our Secure Patient Portal to complete your registration forms online prior to your appointment. Our staff can send you an email invite to gain access to our secure portal. Please bring to your appointment any X-rays, CT or MRI scans. If you prefer not to use our portal, please download and complete the forms below.

For Existing Patients…
Prescription refills may be requested through our Secure Patient Portal. Please make requests at least three days before your medication runs out. This will allow your doctor time to review your prescription needs and complete your refill. Prescription refill requests can only be taken Monday – Thursday.
If you experience negative reactions to your medications, or if the medication is not effectively helping you, please call our office immediately.
